Find Your Inner Parakeet – Liability Waiver & Release Form

By participating in a Find Your Inner Parakeet retreat, you acknowledge and agree to the following:

1. Assumption of Risk
   I understand that activities such as walking outdoors, hiking, art-making, and attending small group retreats carry inherent risks. I voluntarily assume all risks associated with participation, including but not limited to personal injury, property damage, or loss.

2. Health and Safety
   I affirm that I am in good health and capable of participating in retreat activities. I agree to act responsibly and communicate any health concerns to the facilitator.

3. Waiver and Release
   I release and hold harmless Find Your Inner Parakeet, its owner(s), affiliates, facilitators, and property partners from any liability, claims, or causes of action arising out of or related to my participation.

4. Media Release (Optional)
   I grant permission for photos taken during the retreat to be used for promotional purposes unless I notify the facilitator in writing.

5. COVID-19 and Illness
   I understand that participation is voluntary and that I am responsible for monitoring my health. I agree not to attend if I am sick or symptomatic.
